Release 0.8.0: typed rejections and refusals

Package the schema v0.3 rejection/refusal work after the GLM branch merge.

Includes the rejections CLI artifact, MCP summary tool, rejection-derived failures, redaction coverage for rejection evidence, and the Claude alias fix.
